标题: 7月1号以后做过ACS职业评估的来看看
现在万事俱备，在网上已经提交了ACS申请，准备DHL相关材料，但对于ACS申请form有一个疑问，请看这个链接：

2.  Download the relevant application form
Depending on your qualifications, there are two types of Pre-application Skills Assessment (PASA) applications:

* Applicants who have ICT or ICT-related qualification should submit one of the following PASA Application Forms:

      Microsoft Word Icon Form PASA 1.0 (General)
      Microsoft Word Icon Form PASA 1.1 (Professional Members of the ACS)
      Microsoft Word Icon Form PASA 1.2 (Applicants seeking recent work experience exemption)
      Microsoft Word Icon Form PASA 1.2.1 (Applicants seeking recent work experience exemption but wishing to have their professional ICT experience taken into consideration)

* Applicants who have non-ICT qualification, or no qualification, but have extensive experience in ICT should submit Microsoft Word Icon Form PASA 2.0 (Recognition of Prior Learning).

问题1：如果我已经在线申请了，还需要填这个表格呢？
问题2：在ACS在线提交成功以后，可以下载一个PDF文档，这个需要打印出来随着纸质的文档快递过去吗？

1、我们是在线申请的，没有填表格
2、我们把这个PDF文档打印之后签名，然后和纸质文档一起快递过去

An Online application for PASA must consist of:

A copy of the signed online ‘application record’ and the email acknowledgement with the reference number.
A certified copy of a birth certificate or relevant passport pages
A certified copy of academic qualification certificates
A certified copy of academic transcripts
Certified copies of detailed and descriptive employer references
Appropriate fee

我的理解红色部分应该是指这个

是不是说有了passport就不用要出生证明了？

要护照的公证件才不需要出生证明

passport 要提供的是 relevant pages，不是 certificate 啊？怎么也要公证吗？
