户口本上有个曾用名, 但是事实上从来没有用过.表格40上问题18问: Have you been known by any other names? (including name at birth, previous married names, aliases). 要填写吗?
还有问题18里面问原因: 有 Deed poll 和 Marrige. 什么是Deed poll? 我的护照,出生证明里的名字, 都不是这个曾用名, 那么我应该选哪个原因?
