Personal Documents

Please provide a coloured scan of the ORIGINAL birth certificates for both xxxxx,xxxxx

The 887 is the second stage of a two stage visa application process. For this reason, where the
applicants have not travelled extensively since their prior medical used at the UZ496 visa stage

indicate that the following applicants have travelled for 3 consecutive months since their previous
medicals expired:

Please pay careful attention to the information available on this page including . Applicants

Visa applicants in Australia must contact Medibank Health Solutions (formerly known as HSA)
to arrange a health examination.

Applicants outside of Australia must arrange their health examination with a panel doctor and/or
radiologist nominated by the Australian Government. A list of panel doctors and radiologists is
at http://www.immi.gov.au/contacts/panel-doctors. Please note that the panel doctor is required
to send all medical results to the Local Clearance Unit in Australia and not give them to applicants
or their representatives.

Before attending a health examination applicants need to:

1.
Print off the required numbers of forms 26 and/or 160 to present to the panel doctor and
radiologist; and
2.
Complete the “Office use only” box to indicate:

If you are unable to provide some of the required documents by due date (such as medical results
or police checks) for reasons beyond your control, please send us evidence (for example receipts)
that you have applied for these documents.

If you do not respond by due date, or if your response is unsatisfactory or incomplete, then in
accordance with legislation, your application may be decided based on the available information.

Providing Documents

To provide additional documents to the GSM Brisbane office please attach online via link:

The GSM Brisbane office accepts scanned copies of original documents (non-certified) provided
they are in colour. If you scan documents in black and white, then they must be scanned copies
of the certified copies. The valid file types for attaching documents are BMP, DOC, DOT, GIF,
JPG, PDF, RTF, TXT and XLS.

In the event of an error with the online link please email the documents as scanned attachments
to brisbane.gsm.team15@immi.gov.au

You can keep up to date with the progress of your visa application by using the Online Visa
Enquiry service once your case has been allocated to a case officer. By using this service you
can check if we have received documents that you have attached to the browser or sent via email.
You can use your TRN to access this free 24 hour service through our website at
https://www.ecom.immi.gov.au/inquiry/query/query.do?action=eVisa

DO NOT send any original documents or provide paper copies of documents already attached

Certified copies

Copies of documents must be certified as true copies of the originals. 'Certified copies' are copies
of documents authorised, or stamped, as being true copies of the originals, by a person or agency
recognised by the law of the country in which they are certified.



Persons authorised to certify copies in Australia include (but are not limited to):


Justices of the Peace

Pharmacists

Solicitors

Nurses

Police officers

Registered medical practitioners

Bank managers

Postal managers
Documents in languages other than English should be accompanied by an English translation.
If the applicant is onshore, the translation should be undertaken by a translator accredited by
the National Accreditation Authority for Translators and Interpreters (NAATI). Failure to do
so may significantly affect the processing of your application. Translations provided by
non-accredited translators overseas should be endorsed by the translator with their full name,
address, telephone number, and details of their qualifications and experience in the language
being translated. Please note that, where applicable, the copies of both the original document
and the translation must be certified as true copies.

Changes in circumstances

If your circumstances have changed since application lodgement and as a result an answer in
your application or information given to the Department of Immigration and Citizenship is no
longer correct, you must advise us in writing as soon as possible.

Examples of a change in circumstances include:


changes to address and/or contact details

changes to employment eg a new job

obtaining a new passport

new member of the family unit

discovery that information previously provided is incorrect.
You can provide this information to the GSM Brisbane office by letter or email, together with
any accompanying documents. If you are notifying a change of email address by sending an
email from your new address, you must include details of your previous email address, as
evidence that you have authorised the change.

You can also use forms specifically designed to allow you to notify the department of changes.

就是你的彩色出生证明,要彩色的,你以前的黑白的不行。。。不是要你的医院出生证明。。

谢谢LS,出生公证书算不算ORIGINAL?移民官本意是什么?因为之前我给移民局是黑白的扫面件,是不是如LS所说coloured 出生公证的扫面件,就可以了?

如果你的文件是彩色的,这样移民厅接受直接扫描的文档,如果你的文件是黑白文件,这样的话,你要去公正然后扫描才可以被接受。


额~~我发现我的中文语法很有问题。
