以前我申请reissue收到的表和邮件，现在不知道改了没有，你自己也去查查。 填好表，扫描Email过去。
Dear 
If you would like a copy of an assessment results VETASSESS can provide you with a REISSUE. I have attached an application form for a reissue, which you can print out. Please complete the section that shows your new nominated occupation.
A reissue is a reprint of your assessment, with the current date. You must place your request for a reissue of your skill assessment letter in writing with your original signature on it, quoting your file number. A fee of AUD $27.50 per letter is charged for this request.
The relevant fee must be paid in Australian dollars and must accompany the reissue form as a bank draft, bank cheque or money order payable to VETASSESS. Funds can not be paid directly to our VETASSESS bank account.
You can pay your fee to VETASSESS by credit card by providing the attached credit card form. This request will not be accepted via email, or over the phone. You can fax this request to the number below or post it to the address below.
If your case was completed over 6 months ago we will have to retrieve your file from archives. Please allow 3 weeks for this request to be processed.
Please take note that the 3 weeks is an estimate only as it clearly states on our website. Qualifications Assessment Unit are unable to guarantee that an application will be completed within this timeframe.
If there are any changes to your contact details (name, address or telephone number), please inform VETASSESS as soon as possible. This will prevent communication delays.
Your application reference number is XXXXXXX Please quote it whenever you contact this office or send any correspondence to VETASSESS.
